For many, the incest fantasy is just that—absolute fantasy. A huge number of folks with an incest kink have zero desire to manifest this in real life. The taboo of it is a huge turn on, but if sister turned into seductress for real, they would have no interest in following through. Another distinction that is important is consanguine incest versus blended family incest. For many, they are interchangeable.
