1. Ball Busting
This includes direct contact to the testicles from kicking, kneeing, punching, or squeezing. In Japan they call it Tamakeri (ball kicking) and it has a huge community.

2. Ball Stretching
A stretcher will lengthen the scrotum, while pulling the testicles down. If the stretcher is too tight there is a chance that circulation can be cut off. There are various stretcher sizes and they can be made of leather or steel.
3. Ball Crushing
The testicles are places within a metal or plastic crushing device that is tightened with screws to slowly crush the balls.

4. Parachuting
A small collar (parachute), encasing the scrotum, is secured around the scrotum before weights are attached. A man can enjoy the constant sensation of being dragged down as well as its squeezing effect.
5. Humbler
This is a BDSM restraint used on a submissive male. A cuff device clamps around the base of the scrotum and is attached to a bar that is situated behind his thighs just below the buttocks. The man is forced to keep his legs bent as any movement will pull on his constricted scrotum, causing mild to severe pain.
6. Cock Rings
Cock rings are not always used for BDSM torture, but they can be.
This ring-shaped sex toy is secured round the scrotum between the body and the testicles. There may be two adjoining rings/cuffs—one for the base of the penis and one for the scrotum. Some also come with padlocks that can be part of a slave/owner relationship.
Stretching that occurs during sexual intercourse can vary from enjoyable to painful depending on the constriction.

Like the fetish name says, this is a form of torture, so like many BDSM activities, safety must be a priority.
